Dove House Hospice, Chamberlain Road, Hull, HU8 8DH. Please remember to include a note explaining how you raised your money and any sponsorship forms if you have some. Please do not send cash through the post.
Alternatively you can drop cash into the main hospice reception or into your local Dove House shop.
