Referring someone to Dove House Hospice
If you think yourself or someone you know could benefit from the services Dove House Hospice has to offer then it is easy to make a referral and start getting the help the hospice provides.
Who can be referred?
Dove House Hospice can help anyone who is over 18 years old and has been diagnosed with a life limiting illness that is no longer curable. The hospice can also help family and friends who need bereavement support, and as long as there has been a prior connection to the hospice (the person that has died once used our services in some way), then a referral can be for support as well.
Who can make a referral?
Dove House Hospice has an open referral system which means anyone can make a referral on the patient's behalf as long as they have the patients permission to do so. Referrals can be made from the patient them self, their family or friends and can also be made by medical professionals like GPs or district nurses.
How can you make referral?
We've tried to make our referrals process as easy as possible and so all you have to do is phone our Medical Secretaries on 01482 784343 and say that you would like the hospice's help. If you need to make a referral out of office hours the Inpatient Unit can deal with this over the phone however the referral will not be assessed until the next working day unless the case is an emergency when it will be dealt with the same day.
What happens after I have made the referral?
After you have made the referral to the hospice the information you have given will be assessed by the medical team and they will work out which of the hospice services would be most relevant and useful in your case. They will then contact you about coming to Dove House Hospice.
